Membership Application Instructions

To submit your Cy Falls Athletics Booster Club membership application, use the following instructions:

1. Click this link to obtain your username and password (it will be sent to you in an email)
2. If you do not have a username, click this link to create a user account
3. Once you obtain your username and password, click this link to complete and submit your membership application
4. After completing the application, click the button called Submit Membership Application
5. You will be taken to a secure payment screen where you can enter your credit card information and click submit

If you prefer to mail-in your application with a check

1. Click the following link to download the membership application in PDF format (if needed, download free Acrobat Reader)
2. Print out the application
3. Manually fill out the application
4. Send your payment with the application to:
