Be sure to have all paperwork turned in to the athletic trainer prior to the start of try-outs. All required paperwork can be found at www.cyfallssportsmedicine.com under required paperwork. Athletes must have a physical completed by a physician and proper papers signed by parent/guardian before he or she may try-out.
